I think you are taught the toughness feat something like this:

Depends on the trainer. Fred, for example, I am pretty sure messes with my brain till I think I am tough.
But Truth is eternal, so it can never be old or new. It never ‘was’ or ‘will be.’ It just ‘is.’ It always ‘is.’

It's always a shame when the hammer of poor design choices smashes the fun of player tactical adaptation.
Well I don't know about other servers but on Khyber the trainer exposes your build to the entire community of "trolls" that will tell you everything that is wrong with it and how it can never survive in Epic Elite content and how un-optimal it is (most of these comments coming from people that follow builds on the forum and have never examined the whys and wherefores before)... If you can survive that without crying or raging you are then considered "Tough Skinned" and are auto-granted the toughness feat.
With a medicine ball, while being called Butterbean, by an NPC named Izzy Mandelbaum:
It's go-time at 5:00!
By yanking out ALL of your nose-hairs in one go.
If you can survive that, you can survive anything.
The trainer simply places two hands on either side of your forehead and proceeds to suck out all the weakness.
The only thing that remains is toughness.
Trainer: So you want to toughen up, eh?
Adventurer: Yeah, I want people to hit me and not feel it!
Trainer:: You want people to hit you?
Adventurer:: Nooo, stupid! I don't want them to hit me!
Trainer:: You just want them to not feel it...
Adventurer: No, I want me to not feel it!
Trainer: You could try wearing better armor.
Adventurer: I have good armor. I want to be tough!
Trainer: Life is tough.
Trainer: Say that again and I'll hit you.
