Default After a TR, does an F2P have to ...

...run a quest on Normal again to unlock Hard - assuming the character already unlocked Hard on first life?

"In addition, true reincarnation allows a premium/f2p character to open quests on a higher difficulty - the first true reincarnation allows the character to open quests on Hard difficulty, and second and subsequent true reincarnations allow the character to open quests on Elite (VIPs can, of course, open all quests at Elite on any character)."

The difficulty unlock appears to be independent of whether or not you completed the quest on a previous life.

Yes. You lose all quest standings, flags, and accumulated favor (although you still keep anything server-related that you unlocked via favor like classes/races/etc. You lose your character-related stuff like bank slots and inventory slots, though.)

One of the main perks of being VIP is being able to run quests on whatever difficulty you want.

Ooh, that's neat! I missed it when that got implemented.

The same time as (and because of) bravery bonus.
