There's a few inconsistencies in the guide. The order and level feats are taken in doesn't match perfectly with the order the feat explanations list.

Looks pretty good. I'd take out the mention of Weapon Finesse in the Important Abilities section, though. On a Rogue or Monk 101? Sure. But the Fighter class is probably the most ill-suited for DEX-based after Barbarian. It'll only confuse new players into thinking that Finesse might be a good idea for their TWF Fighter.
There's 4 toughness feats taken in the level-by-level breakdown (at 1, 12, 15 & 18), only two of which are listed in the feat explanations (levels 1 & 12).
Oh, and Oversized Two Weapon Fighting isn't mentioned at all in the feat explanations. You could probably throw that in with TWF, iTWF, and gTWF feats I guess.
well spokenIn DDO, playing a pure sword and shield fighter – otherwise known as an "AC Tank" – becomes very challenging at higher levels. This is because AC doesn't scale as high as attack bonus at character levels above 10, making it imperative for AC tank players to gather every tiny bit of additional AC they can find. Additionally, investing in AC boosts like a shield can reduce a fighter's damage-output. In Fighter 101, we'll be covering the more damage-focused fighter styles like two-handed fighter and two weapon fighter, as AC-focused fighters are a whole guide unto themselves.
